Who are we?

The District Nursing team provides a variety of nursing care interventions to adults aged 18 years and over who meet the eligibility for home visiting. A person may be considered for a home visit if:

  • It would be detrimental to their condition to attend a local clinic
  • Their medical condition affects their mobility affecting their ability to travel
  • Treatment requires a clinician to assess the environment in which they live

Once the individual is well and fit enough to do so they will be asked to self-care or continue their care in a clinic based service. The team receive referrals from GP’s, Carers, Hospitals, relatives, social services and self-referrals. All referrals are reviewed and you may be signposted to more appropriate services to meet your needs.

Examples of nursing care interventions include:

  • Assessment and care planning
  • Wound and leg ulcer care
  • Pressure ulcer prevention and care
  • Bowel and bladder care including continence assessment and support with catheter management
  • Care at the End of Life/Palliative care.

Each staff member will have an identity badge, please ask to check this before you allow them in your house. The team work as part of a multidisciplinary team in conjunction with other community services to provide co-ordinated and integrated care that is compassionate and responsive to patient needs, values and expressed preferences. The team will work to support you to stay at home, maintain independence and achieve self-care where possible.

How and when can I be seen?

The District Nursing team will care for you in your own home or care home. We are not able to give specific appointment times however we aim to work with you on an individual basis to agree a convenient time slot for your care.

What can you expect from the service?

The District Nurse will assess your individual health needs in partnership with yourself to identify and plan the nursing care you require. This will involve answering some health related questions and agreeing a personalised care plan with you. Your personal information will be kept secure, however, we may need to share some information with other professionals to help care for you.

How you can help us?

If you need to cancel a visit please let the District Nursing service know or leave a message. We may ask you to keep your pets in a secure separate room when caring for you. We will ask you not to smoke during visits. You will be given some District Nursing paper notes to keep at your home. These should be kept in a safe place and returned to the District Nursing Team at the end of your episode of care. Your co-operation would be appreciated as we support healthcare professional training and your consent will be sought if a student will be visiting you with us.
